If you have had a vision screening before by a Colorado Lions Club listed below, click on the name of the club and email the form to them. 

If you have not used the Kidsight Colorado Vision Screening Program before, email the saved form to  ExecutiveDirector@KidsightColorado.org.  

You will be contacted shortly by a Colorado Lions Club to schedule a Vision Screening event.
